I am putting this matter in suspense for another three weeks I don't think I could get away with taking more time than that.

This office has been notified of defective material received by
General Services Administration Supply Distribution Facility,
Chicago, Illinois, which was supplied on contract GS-OOS-29058.

The defective materials involved are 542 flat top desks, NSN

7110-00-113-2832, received on purchase order C-w-91981-6. Totalul 672 2232

These desks were inspected in accordance with MIL-STD-105D and found unacceptable. The defects found include bare metal, a misaligned desk top, poor paint finish, and a chipped formica top.

Please notify this office when you will repair or replace the above asks. If replacement will be made, provide disposition instructions for the defective material located in our Chicago, Illinois depet. Please reply within five days from the date of receipt of this notification and refer to control number 2QC-CO2-75 when replying to this letter.

We checked our records and find that we never enjoyed a contract under the number GS-OOS-00214.

In addition, we never made shipment of this type of item, namely a
NSN 1269 to the V. A. Hospital in Hines, Illinois.

It appears GSA gave me the wrong contract number. When they come back to us with another letter with the correct contract number, I will then have to authorize local repair.

Tera Homolka of the Veterans Administration Hines Hospital, Hines, Illinois 60141 has received one defective desk, NSN 7110-00-105-1269, manufactured by your firm under contract GS-00S-00214. The purchase.. order number is not available.

This desk is not suitable for use because it has sharp edges and

The fabric is pulling away from the partition.
