Dealer to prepare Form 090-MMP per captions, and forward with re-
mittance to supplying Terminal on specified due dates, Meter test
gallons deduction on Form 090-MMP are to be supported by attaching
meter test form to duplicate copy of 050-MMP.

B. At time of a price change, the Dealer is to immediately record pump meter readings and prepare Form 090-MMP, using the previous tank wagon price and mail remittance at once.

C. Dealer to record changes in price, taxes, rent, etc., on Dealer Price Record, upon notification by Salesman or District Office.

Dealer to prepare additional 090-MMP prior to noon on last calendar
day of each month and forward with remittance to reach Terminal on
the morning of the first day of the subsequent month.

E. Remittance and Forms 090-MMP, together with Forms 283 and 090-D, are to be forwarded to the Terminal using addressee postage due envelopes · furnished.

F. Remittance may be by Dealer's personal check, money order, Form 090-D credit card tickets, 574 debit or credit memos, or any combination of these means. No portion of any remittance made by mail shall include cash, currency or third-party checks.

G. Original Forms 090-MMP are to be retained by the Dealer and duplicate' and triplicate copies mailed to the Terminal. If it is necessary to void any 090-MMP invoices, mail all copies to the Terminal.

DEALER END-OF-MONTH INVENTORY Exhibit C-Form 588

A. Gauge stick readings and the gallons they represent must accompany each end-of-month remittance. Gauge stick readings must be taken at the same time pump meter readings are taken for preparation of end-of-month 090-MMP.

This end-of-month inventory is to be submitted, in duplicate, to the Terminal on end-of-month Dealer Inventory Report. This inventory form should accompany each end-of-month remittance.

TOTALIZER FAILURES - EQUIPMENT CHANGES

A. Totalizer inaccuracies or failures not immediately noticeable to the Dealer will become apparent to our Terminal in connection with the maintenance of Terminal records. When this occurs, Auditor will conduct a complete audit and arrive at mutual agreement concerning andakanad pallama sald

3. TOTALIZER FAILURES

At other times, totalizer failure will become immediately apparent
to the Dealer. When discovered, Dealer should immediately notify
Terminal supply point. Arrangements will be made to conduct an
audit and to mutually arrive at settlement for unregistered gallons sold.

When necessary to replace a totalizer, computing head, or entire pump
unit, the Dealer will prepare, in full, a Form 090-MMP to record the
final reading of the old totalizer and on a second Form 090-MMP
record the opening reading of the new totalizer. Both readings must
be properly identified and attested on the forms by the Construction
and Maintenance Department Representative.

DEALER PERFORMANCE

Detailed and current records are maintained at Terminal supply point to assure prompt and accurate performance on the part of Dealer. Any departure from normal performance becomes immediately apparent. To avoid unnecessary time on the part of our Sales Representative or Terminal, or embarrassment to Dealer, Dealers are reminded that it is most essential that they adhere to:

A. Prompt preparation and mailing of remittances on scheduled remitting dates.

B. Prompt preparation and mailing of end-of-the-month remittance and closing inventory.

Accurate totalizer readings and reporting of all gallons sold
at the time of preparation of each Form 090-MMP.

Accurate and prompt preparation of cut-off 090-MMP at the time
of price changes.

5. HOW TO PREPARE 090-MMP (SAMPLE ATTACHED)

At the top of the 090-MMP, write in the date and time that pump
totalizer readings are taken. Indicate whether A.M, or P.M.

On your remittance day, read all pump totalizers. Disregard
tenths of a gallon. Write totalizer figures in the spaces provided
on the left side of the 090-MMP ticket. Please write these
legibly. Your pumps are all numbered; make sure totalizer readings
are posted properly. SUPER PREMIUM pumps carry even numbers;
AMERICAN REGULAR are odd numbers.

Add up both columns of meter readings. Show the total of each
in the "Summation Line." Check your addition.

On the line, "Less Previous Reading," show the last total meter
readings by brand taken from your last previous sales ticket. In
order to do this, it will be necessary for you to have the copy
of this last sales ticket on hand.

readings and show in the "Total Gallons Withdrawn" line.

If a maintenance man has tested your pumps, he will leave you a signed
form verifying the gallons that have been run through your pumps, If
this happens, write these gallons in the space noted "Less Meter Test
Gallons" and subtract from the total gallons. You must submit this
form along with your remittance.

H. Carry the gallons you owe for by brands, to the proper space in the upper portion of the form, Recheck for accuracy.

In the column captioned, "Total Price and Gal Taxes," show your
purchase price by brand and include in this price Federal Tax and
State Tax.

Extend this by multiplying gallons by brand and show in the amount
column at the right. Check your extension.

In the rent line, show the total gallons involved, both PREMIUM and
REGULAR. At the right of this, show your rent per gallon. Extend
this by multiplying by your rent per gallon and show the total rent
owed in the column provided. Check your extension.

If you have a liquidation allowance, handle exactly as you did in
the rent line above. Check extension.

Add the total of the rent and liquidation and show the total of
these in the right column. Check addition.

Add your total product cost and your total rent and liquidation cost
and write this figure in the third total space provided.

Write the 283 numbers on the line provided. Add all Forms 283 and
show your total on the same line in the extreme right column.

Q. Subtract the credit 283's total from the above total. Check subtraction.

If on some previous ticket you have made an error, a Form 574 Over or
Short, whichever the case, will be mailed to you. When you receive
one of these Over or Short Forms, you will enter it in the lower
portion of the sales ticket on the proper line. Write the number of
the form and show the amount. Subtract or add, whichever the case,
and write the amount that you owe in the line captioned, "Total
Amount Owed by Dealer." Check arithmetic.

S. Sign the form in the lower left-hand corner provided for Dealer signature.

Please write meter readings and all figures as legibly as
possible. In order to make good clear carbons, we suggest
that you use a hard lead pencil or preferably a ball-point pen.

A supply of envelopes and forms will be maintained at supplying
Terminal. Request additional supplies from Terminal.

At the end of each month, you are asked to furnish us,
by brand, the exact gallons in the storage tanks. You have been
provided a separate form for this purpose only. At the same time,⠀
you are asked to read the meters on each pump and make a
remittance in addition to your regular scheduled day.
